Predicting amyloid positivity and tau burden in dementia using a
combination of different biomarkers

Purpose: Amyloid status and tau burden are essential factors in determining the effectiveness of anti-amyloid
treatments for Alzheimer's disease. This work aims to predict amyloid positivity and tau burden from different
combinations of biomarkers.
Materials and methods: In this study, a total of 37 CU, 77 MCI, and 105 Dementia were included with T1-MRI,
[18F]Florbetapir, [18F]Florzolotau, plasma biomarkers (i.e. pTau217, pTau181, Aß42/40, GFAP, and NfL), and
neuropsychological testing. Amyloid positivity (A+/-) from visual reading was applied, and in addition the amyloid
Centiloid (CL) was calculated. The global tau SUVr was generated within a data-driven AD-vulnerable VOI to derive
tau burdens (low, intermediate, high) using the Gaussian mixture model from a cohort of CU A- and dementia A+.
The comparison of each biomarker value among different tau burden groups was performed. The logistic
regression analysis was conducted to predict the amyloid positivity and tau burden.
Results: In dementia/MCI with A+ cases, a significantly higher CL (low= 44.32±28.73; intermediate= 68.24±37.08;
high=96.81±15.48) was observed with increasing tau burden. The level of pTau217 is significantly higher in the high
tau group as compared to the low and intermediate tau group (Figure 1). For the prediction of amyloid positivity,
pTau217 as a predictor performs well for classifying A+ and A- (AUC=0.93; R2=0.54; p
